  • School supply lists can be found by visiting the "For Students" section your school's website. Use the "Schools" drop down menu at the top of the page to access your school's site. Please note: students in Grades 9-12 will receive their supply lists from their teachers on the first day of class.

  • You can now verify, update, and create your student information online for the 2016-2017 school year. Sycamore Community Schools uses Final Forms, an online student/parent online forms registration tool. If you used Final Forms in 2015-2016, you simply need to verify that the information is still correct and make any necessary changes. If you have not used Final Forms in 2015-16, you can create a parent account and fill out the online forms for yourself and your children. If you are new to the district, you can use Final Forms to submit new student registration information. New families need to make an appointment with the district registrar.     *Please note - The application will NOT be mailed to every home this year. If you are unable to apply online - You can obtain a copy of the form via:

    • The Child Nutrition Page
    • Your child's school office after August 10
    • Sycamore District Offices during business hours (8:00-4:30)

    Interested in fee waivers? If you qualify for free or reduced-price meals, you are eligible for certain program fee's to be waived. Be sure to mark yes to all the waivers you are interested in receiving. Failure to do so will result in your student being charged these fees.
