• Go to: sycamore-oh.finalforms.com

• Locate the Parent icon and click New Account.

• Enter your name, date of birth, and email address, then click Register.
  Note: You’ll receive an email within 2 minutes prompting you to confirm and complete your registration. If you don’t receive it, check your spam folder. If you still can’t find it, email support@finalforms.com.

• Open the account confirmation email from FinalForms. Click Confirm Your Account in the email.

• Create your new FinalForms password, then click Confirm Account.

• Once logged in, click Register Student to begin adding your first child.
  (See additional instructions below for registering your student.)

To register your student, you will need the following documents:

• Student’s birth certificate

• Immunization record

• Proof of residency

• Parent/guardian photo ID

• Court-stamped custody documentation (if applicable)

• Create a parent account using the steps listed above.
• Click the Parent icon and log in to your account.
• Click Add Student.
• Enter your student’s legal name and other required information, then click Create Student.
• If your student plans to join a sport, club, or activity, select the appropriate checkboxes and click Update.
  • You may update selections anytime before the registration deadline.
• Complete each form and enter your full name (e.g., “Jonathan Smith”) in the parent signature field. After signing, click Submit Form and continue to the next one.
• Once all forms are complete, you will see a “Forms Finished” message.
  • Note: If required, your student will receive an email prompting them to sign any student-specific forms.
