Final Forms
Sycamore Community Schools partners with Final Forms to provide a one-stop shop for all electronic forms including emergency medical, transportation, Computer Agreement forms, etc. For new families, you will only need to fill out the forms once and then update and verify the following year. If you are not new to the district, please log in to Final Forms to verify that the information from last year is up to date and add your electronic signature to each form.
Signing Up For Final Forms
- Go to Final Forms
- Click NEW ACCOUNT under the Parent Icon
- Type your NAME and EMAIL and then click REGISTER
- Check your Email and click CONFIRM YOUR ACCOUNT in the email text NOTE: You will receive an email within 2 minutes prompting you to confirm and complete your registration. If you do not receive an email, check your spam folder. If you still do not see the FinalForms email, please email support@finalforms.com
- Type your password and click CONFIRM ACCOUNT
- You will be asked if you want to grant another Parent/Guardian access to your registered students.
- Either click SKIP THIS STEP or type Name and Email Address and click ADD PARENT ACCOUNT.
- Your account will be created, you can then REGISTER STUDENT for your first child.
