Immunization Requirements for 2017-2018 School Year

2017/2018 Immunization Requirements


Dear Parents of Students at Sycamore Community Schools,


We are reaching out to make you aware of changes in the immunization requirements for children entering 7th and 12th grades in the 2017/2018 school year.  In December of 2015, The State of Ohio made it a  requirement for all incoming 7th graders to have (1) dose of MCV4 (meningococcal) prior to their entry this August 2017.  The MCV4 is a new addition to the Tdap (Diphtheria, Tetanus and Pertussis) booster that has been required. Additionally, students entering grade 12 will be required to have (2) doses of MCV4 prior to August 2017 unless the initial dose was given on or after the 16th birthday.


Because this is a statewide mandate, it may be difficult to schedule appointments in the late summer. Please help us make this grade transition as smooth as possible by checking with your physician to verify your child is up to date on immunizations, and making an appointment as early as possible if your child needs to be updated.


Once your child has received the immunizations, turn in a copy to your current building nurse (contact information below). The record may be sent with your child, emailed, faxed or mailed. Verification of your child’s immunization record must be on file with Sycamore Community Schools.  Those students who do not have proof on file and who have not met immunization requirements by the first day of school will be excluded within 14 days from the beginning of the school year (Ohio Revised Code 3313.671).  We are asking that the updated shot record be submitted to Julie Wolf, School Nurse at E.H. Greene if your child is in the 6th grade. If your child is at the High School please submit it to Susan Murphy, School Nurse. Please submit the document by April 28th, 2017.  Again, students without the appropriate documentation will not be allowed to enter school in the fall.
