Second Semester Change Requests Now Being Accepted

We encourage families to remain in their current learning model for the second semester for stability and consistency in teaching and learning as well as for the safety and health of our students and staff. However, we understand that in some instances, that learning model may not be working for their student. Beginning today, we will accept requests to change your student’s learning model. 

 

DO NOT fill out this form if you:

  • plan on keeping your student in his/her current learning model. Students will be automatically re-enrolled in their current model unless a request form is submitted by November 30. 
  • have a child in our preschool program. 


If you would like to request a change for the second semester:

  • Please complete a separate form for each student in your household for which a change is requested. 
  • Please designate one person per household to complete this form to help us avoid duplicate requests for the same student. 
  • The deadline to submit a request for a change in model is Monday, November 30 at 10:00 p.m. If you do not complete a form by the deadline, your child(ren) will be automatically be re-enrolled in their current learning model.
  • Request submitted after the November 30 deadline will not be accepted.
  • Families will be notified of their change in status prior to Winter Break.
